How Care Management Service Works:

Care Management Service begins with a family consultation. The consultation includes a comprehensive assessment of a client's and family's needs. A plan is then developed to provide appropriate care and support for the client. The family may then choose to initiate and oversee care provision on their own, or obtain ongoing care management services to include:

Arranging for and monitoring care and other support services.
Coordinating services with doctors and other care providers.
Contacting client regularly and keeping the family informed.
Finding an alternative living situation if needed and coordinating the client's move.
